812 - Unchecked checkboxes appear checked in Customize favorites toolbars UI

Steps to reproduce:
1. Open 'Customize favorite toolbars' dialog box
2. Click the dropdown menu for the plus sign just above the lower-left command list area
3. Select 'Separator' from the dropdown menu
4. Uncheck the checkboxes for 'In menu' and 'In toolbar'
5. Click 'OK'
6. Open 'Customize favorite toolbars' dialog box
Issue: Notice that both of the checkboxes unchecked in step 4 now appear to be checked (although they are still unchecked internally, which is good).
