For example, the "copy file path" command has no icon and you can not assign an existing icon or an .bmp file as an icon.
You can pin the command to the toolbar but you can not see what the command is.

Also "show hidden files" [also known as "toggle visibility of the hidden files"] has no icon

And I use it quite often

For each command you can define own icon. Check for Details for a toolbar item
